Day Break Adult Day Services: An Individualized Choice
Day Break programs provide a safe, nurturing environment supervised by professional staff for the person who may need day time care to remain at home. Individual care plans, matching personal needs with appropriate services, are developed to promote independence and choice.
Admissions, tours, or a home visit can be scheduled within 48 hours, if needed. Admissions are coordinated by the Day Break Team, working with your family and physician.
St. John's Senior Communities offers both a medical model program, located at St. John's Home, and a social program, which is based at St. John's Meadows.
